Birth Certificate Name Change in Punjab

One application, so every mistake has to be fixed together.

Punjab corrects birth certificates through Sewa Kendras on the Health Department form, with statements from two credible persons, and the declaration says no further correction will be made later. We check every entry with you and prepare one complete file.

In short

A birth certificate name change in Punjab is filed through a Sewa Kendra on the Health and Family Welfare Department’s form, for a government fee of ₹10 plus a ₹100 facilitation charge. It needs statements from two credible persons, such as a Sarpanch, municipal councillor or Gazetted Officer. The self-declaration on the form says no further correction will be made afterwards — so get everything right in one application, because there’s no second attempt.

Who can apply for a birth certificate correction in Punjab

  • The person named on the certificate, once they are 18.
  • A parent or guardian, for a child under 18.
  • A close relative or someone authorised in writing, where the applicant cannot attend. Ask the office what authorisation it accepts before sending anyone.

The registrar checks that the applicant is connected to the record, so carry ID that shows the relationship as well as the documents listed below.

Two routes for a birth certificate name change in Punjab: a correction filed with the Registrar, through a Sewa Kendra, or a legal name change through an affidavit, a newspaper notice and the Gazette of India.
A correction fixes what was recorded. A different name is a legal name change, and that needs the Gazette.

Why people correct a name on a Punjab birth certificate

  • A spelling mistake made when the birth was registered, often found years later by a passport office or an employer.
  • No name on the certificate, because the birth was registered before the child was named.
  • A short form or pet name entered instead of the full legal name, or an initial that was never expanded.
  • Marriage or divorce. A surname taken or dropped later is a legal name change, not a correction; see returning to a maiden name after divorce.
  • Adoption, religious conversion or gender transition, each of which has its own route.
  • A mismatch with Aadhaar, school or board records, where one document has to be brought into line with the rest.
  • The date of birth is wrong too, which is a separate application; see correcting a date of birth.

The reason decides the route. A mistake in what was recorded is a correction. A move to a different name is a legal name change, and that needs the Gazette.

How a birth certificate correction works in Punjab

The Health and Family Welfare Department’s correction form covers the child’s name, the father’s, mother’s and grandfather’s names, gender, place and date of birth, and addresses, each given as it is now and as it should be.

Every name is entered in both English and Punjabi, so check the correct spelling in both scripts before you apply. The form is submitted at a Sewa Kendra, and eSewa Punjab, esewa.punjab.gov.in, is the state’s online service portal. The office that registered the birth still has to approve the change, and it is the heart of any birth certificate name change in Punjab.

Why you get only one correction in Punjab

The self-declaration on Punjab’s correction form states that no further correction will be made to the entry afterwards. Most people spot one mistake on the certificate and miss the others. Treat the application as your only chance: list every mistake, not just the one you noticed first, and make sure the spellings match your Aadhaar and school records.

Applying at a Sewa Kendra or online

Punjab corrections are filed at a Sewa Kendra on the Health and Family Welfare Department’s own form, and eSewa Punjab is the state’s online service portal. The service is also listed on the National Government Services Portal.

The application is processed for the district where you currently live, and the office that registered the birth still has to approve the change.

Documents required for a Punjab correction

The Department’s form lists these as mandatory:

  • ID and residence proof of the applicant, such as a voter card, Aadhaar, passport or driving licence
  • The original or a self-attested copy of the birth certificate
  • A school certificate, passport, driving licence, Aadhaar or voter card of the child, father or mother, where their name is being corrected
  • A self-declaration describing the reason for the correction
  • Statements from two credible persons, such as a Sarpanch, Panch, municipal councillor, MLA, MP or Gazetted Officer
  • The hospital or nursing home report, for a birth in an institution
  • Proof of residence and relationship, such as a voter card, passport, Aadhaar, driving licence or bank passbook

For a minor, the declaration is signed by the father, mother or another adult relative. If you no longer have the certificate, get a duplicate first.

Adding a name that was never entered in the register

If the certificate was issued without a name, the Registration of Births and Deaths Act lets the registrar add it free within 12 months of registration, and for a small fee up to 15 years. Ask the office about its current fee.

Why Chandigarh is handled separately

Chandigarh is a Union Territory with its own administration… a small distinction that costs a surprising number of applicants a wasted trip to a Sewa Kendra. A birth registered there is not handled by Punjab’s Sewa Kendras. Apply to the registrar in Chandigarh that recorded the birth.

Changing to a completely different name in Punjab

A legal name change runs in a fixed order: affidavit, newspaper notice, Gazette notification, then the registrar.

Step 1: the affidavit

The affidavit gives the old name exactly as it appears on the certificate, the new name, the date and place of birth, the parents’ names and the reason for the change, sworn on non-judicial stamp paper and attested by a Notary Public. Punjab’s correction form records every name in both English and Punjabi, so settle the spelling in both scripts before you swear the affidavit, and match Aadhaar letter for letter.

Step 2: the newspaper notice in Punjabi and English

Publish the change of name in a leading daily circulating in your district, giving the old name, the new name and your address. Keep the original printed page, because the Gazette office needs the paper itself rather than a scan. Most applicants in Punjab publish twice, once in a Punjabi daily and once in an English one, since some departments ask to see both.

Step 3: the Punjab Gazette or the Gazette of India

Punjab’s own Gazette runs on a weekly cycle with a hard cut-off. The Department of Printing and Stationery publishes the Punjab Government Gazette and puts it on the e-Gazette link of the Punjab Government Portal. Its printing manual sets the timing: matter for the weekly Gazette must reach the Government Press, Chandigarh before 12 noon on Tuesday, and anything arriving later goes into the following week’s issue. Notifications for the Extraordinary Gazette must be in by 2 p.m., or they are published the next day.

That workflow is built around notifications sent by government departments, so a private name change is more straightforward through the Gazette of India, which the passport office, banks and departments across the country accept. Published Punjab notifications can be viewed on the state’s e-Gazette report.

The Department of Publication’s guidelines ask for:

  • A signed undertaking giving your old and new name, your father’s or husband’s name and your address
  • The original newspaper notice
  • The change-of-name matter in the prescribed format, typed, in duplicate, signed in your old name with two witnesses
  • A CD with the same matter in MS Word, without the witness portion
  • A signed certificate that the printed copy and the CD copy match
  • Two self-attested passport photographs and a self-attested photo ID
  • The publication fee, paid through the Bharatkosh portal

The fee is ₹1,100 for an adult and ₹1,700 for a minor. Send the application by post, or hand it in, to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. The office does not accept applications through agents or advocates, so the applicant signs and submits it. Documents should be less than a year old, and nothing sent is returned.

No printed copy is posted to you. Publication usually takes 30 to 45 days; after that, download your notice from egazette.gov.in by searching the weekly Gazette, Part IV, for the week it appeared.

Step 4: back to the registering office

Take the Gazette notification, the affidavit, the original newspaper notice and the existing certificate to the registering office. The registrar decides whether the entry can be updated. Where it cannot, keep the Gazette notification with your birth certificate as proof that both names belong to the same person.

What a name change costs in Punjab

Government and publication costs are modest, and a correction that does not need the Gazette skips the newspaper notice too.

ItemTypical cost
Affidavit (stamp paper and notary)₹200 – ₹500
Newspaper notice₹300 – ₹1,000 or more, depending on the paper
Gazette of India notification₹1,100 for an adult, ₹1,700 for a minor
Correction at a Sewa Kendra₹10 government fee plus ₹100 facilitation charge

If you’d like us to prepare the file, we quote our fee in writing after reviewing your documents, so you know the full cost before anything is filed. It’s a small application fee, but the two credible persons’ statements are what actually take time to arrange. A Gazette application is signed and sent by you, as the Gazette office requires.

How long a Punjab correction takes

RouteTypical time
Correction through a Sewa KendraDepends on verification by the registering office
Legal name changeAround 2 to 4 months; the Gazette step alone is usually 30 to 45 days

Special cases: children, marriage and applicants abroad

  • Marriage. Marriage does not change a birth certificate. A surname taken on marriage is a legal name change, not a correction; the passport office accepts a marriage certificate, but most other departments want the Gazette notification. See our guide to name change after marriage.
  • Children. The father, mother or another adult relative signs the declaration for a minor.
  • Applicants living abroad. The name-change deed can be attested by the Indian Embassy or High Commission and a Gazette of India application sent by post. A family member can usually deal with the Sewa Kendra and registrar in Punjab on your behalf with written authorisation; ask the office what it accepts.
  • Gender transition. The law on changing name and gender on official records was amended in 2026. Our guide to birth certificate name change for gender transition explains the current routes.
  • Religious conversion. A new name taken on conversion follows the legal name change route; see name change after religious conversion.
  • A parent’s name is wrong rather than the child’s: see correcting a mother’s or father’s name.

If your Punjab application is stuck

Ask at the Sewa Kendra for the status using your application number. If the correction is still not moving, take it to the District Registrar of Births and Deaths.

Aadhaar first, since eSewa Punjab, banks and the passport office all lean on it as the default check now. Fix it once there and you won’t spend the next year explaining the same correction at every counter you visit.

Common mistakes that delay Punjab applications

  • Correcting one mistake and missing others, when no further correction will be made later.
  • Leaving out the two credible persons’ statements.
  • Spelling a name differently in English and Punjabi, or differently from Aadhaar.
  • Applying in Punjab for a birth registered in Chandigarh.

If your Punjab application is rejected

Because the form only allows one correction, Sewa Kendra staff tend to scrutinise an application closely before accepting it, and a fair number get handed back at the counter rather than filed and rejected later. The two most common reasons are a name that’s spelled one way in English and another way in Punjabi on the same form, and a credible person’s statement that doesn’t clearly identify who they are — a Sarpanch or councillor’s name alone, without their designation or ID, often isn’t enough. The good news is that this kind of counter rejection doesn’t use up your one shot; the self-declaration’s no-further-correction rule applies to a correction that has actually been entered, not to a form that was never accepted in the first place. Fix the mismatch, get a proper statement from your second credible person, and resubmit.

Example scenario

An illustration of how the rules above work in practice, not a client record.

A family in Ludhiana went to their Sewa Kendra to fix their son’s father’s name, misspelled at registration. They brought a statement from their ward’s Panch as one of the two credible persons required, but the second statement, from a family friend who happened to be a retired Gazetted Officer, didn’t mention his designation or attach any proof of it. The clerk wouldn’t accept the form as complete and sent them home to get it redone properly. They came back a week later with his service ID copy attached to a fresh statement, and the correction was processed without further trouble. Since the form was never accepted the first time, they hadn’t burned through their one allowed correction — only a week.

Birth certificate name change in Punjab: FAQs

Where do I apply to correct a Punjab birth certificate?

At a Sewa Kendra, on the Health and Family Welfare Department’s correction form. The office that registered the birth approves the change.

What does a correction cost in Punjab?

A government fee of ₹10 plus a facilitation charge of ₹100, as shown on the Department’s form.

Who can be one of the two credible persons?

The form lists a Sarpanch, Panch, municipal councillor, MLA, MP or Gazetted Officer.

Can I correct my certificate again later?

Plan on no. The self-declaration on the form states that no further correction will be made to the entry afterwards, so include every mistake in one application.

Is a Chandigarh birth handled the same way?

No. Chandigarh is a Union Territory, so apply to the registrar in Chandigarh that recorded the birth.

Do I need a Gazette notification for a spelling mistake?

No. A correction goes through the Sewa Kendra with supporting documents. The Gazette is for a legal change to a different name.

Does correcting my birth certificate update Aadhaar or PAN?

No. Each is a separate record. Update Aadhaar first, then use it with the corrected certificate or Gazette notification for the others.

Who can apply for a correction in Punjab?

The person named on the certificate, once they are 18. For a child under 18, a parent or guardian applies. Where neither can attend, a close relative or someone authorised in writing can usually file, but ask the office what authorisation it accepts first.

Should I use the Punjab Gazette or the Gazette of India?

The Punjab Government Gazette is published by the Department of Printing and Stationery, and its weekly issue closes at 12 noon on Tuesday at the Government Press, Chandigarh. That cycle is built around departmental notifications, so most private name changes go through the Gazette of India at ₹1,100 for an adult and ₹1,700 for a minor.

What if I no longer have the original certificate?

If you no longer have the certificate, ask the registering office for a duplicate before filing a correction; the correction is made on the record, and you need a copy to show what is wrong.

Do I have to use an agent?

No. Every step can be done by you directly with the registrar, and the Gazette office in Delhi does not accept applications through agents or advocates at all: the applicant signs and submits them. People use us to get the file right the first time, not because the law requires it.

When does the Punjab Gazette close each week?

Matter for the weekly Punjab Government Gazette must reach the Government Press, Chandigarh before 12 noon on Tuesday. Anything received later is carried to the following week. Extraordinary Gazette notifications must arrive by 2 p.m. or they publish the next day.

Which newspapers should I use in Punjab?

One Punjabi daily and one English daily circulating in your district. Keep the original printed pages, because the Gazette office needs the paper itself rather than a scan.
